Long term & respite care

Our long-term care services are available for individuals who can no longer live on their own or in an assisted living center. We focus on maintaining our residents’ quality of life and enabling them to keep their sense of independence. Guggenheimer Health and Rehab Center offers a range of long-term services including:

24-hour nursing care

We have highly skilled nurses who are available around-the-clock to assist with any medical circumstances that arise. Our nurses are more than just professionals; they are warm and caring and provide our residents with a sense of positivity and safeness.

Plan of care

Various members of our staff team work together to create a personalized plan of care for each resident. The resident and his family’s priorities are taken into account and incorporated to ensure that the plan meets the expectations of all involved in the resident’s care. We set mental, physical, and social goals and design a plan to achieve them.

Alzheimer’s and Dementia unit

For residents facing Alzheimer’s and/or dementia, we have a secure and safe unit operated by specially trained staff. We offer special programming for these residents which is specifically geared to their needs.

Recreational Services

Guggenheimer Health and Rehab Center is not only focused on the physical health and medical well-being of our residents; our residents’ emotional satisfaction and cognitive stimulation is of equal priority. Therefore, in addition to offering top quality clinical care.

Dining Services

Our residents are served three meals a day, as well as snacks throughout the day. Our chef caters delicious meals while taking care to accommodate for allergies and preferences.

Transportation

We provide transportation for our residents, whether they have an appointment or a family gathering. We have wheelchair accessible vans and buses to ensure optimum safety.

Access to Amenities

To ensure premium comfort and relaxation for our residents, we provide access to many amenities such as WiFi, housekeeping, and laundry.

Short Term Respite Care

Our short-term services are geared towards those who would like to stay in our facility for a temporary period of time. Sometimes, your loved one needs a change of routine, or his or her caretaker needs a break. Whatever the case may be, we welcome residents for any period of time (minimum of four days), whether it be a week to several months.
